A 36 year old female who is a resident of Narketpalli housewife by occupation came with chief complaints of  1. Fever since 7 days  2. SOB since 4 days  HOPI -  Patient was apparently asymptomatic 7 days back then she developed fever on 9th of August which was intermittent in nature, associated with headache, chills and rigors . Also she has complaints of pins and needle like sensation on the left side of her body . ( went through mastectomy of left breast)  Then since Friday she has SOB after having any meal.  ( grade 3 )  And had 4 episodes of vomiting which had food contents , non projectile, non bilious.  PAST HISTORY-  No history of Diabetes , Hypertension, thyroid abnormality, epilepsy, CAD , TB  FAMILY HISTORY- fathers sister has blood cancer .  SURGICAL HISTORY-  Mastectomy-left breast  DR

Rollno.61 Aruna.J Case Report: A 65YR old male resident of Nalgonda a retired farmer by occupation with Chief complaints of slurred speech and weakness of Right upper and lower limb since 3 days. HOPI: Patient was apparently asymptomatic 5yrs ago. Patient was asymptomatic 5 yrs back when he had  Right flank pain radiating to groin, fever, weakness and polyuria and went to hospital and was diagnosed with CKD . He also complains of SOB on exertion since past 5 years. He Had  10 rounds of dialysis for 2 years but then he was advised to stop the dialysis and was normal without dialysis since 2 years. H/O traum

Pedal Edema since 3 months  2. Shortness of breath since 3 months  HOPI   Patient was apparently asymptomatic 12 years back .  He was having complaints of vomitings and chest pain and was diagnosed with Ischaemic Heart disease and went through bypass surgery (CABG) .  The same time he was diagnosed with DM and Hypertension.  3 months back was having pedal Edema which is pitting type and extends till knees . shortness of breath on walking a long distance for which he went to hospital and was diagnosed with kidney disease.  He came here 4 days back for dialysis. He had 2 rounds of dialysis till now .  PAST HISTORY-  A known case of Diabetes Mellitus and Hypertensio

Vomiting since 15 days  2. Burning micturition since 15 days  3. Right flank pain since 15 days  HOPI: Patient was apparently asymptomatic 15 yrs back.Then he developed pain in the right loin and was diagnosed with renal calculi(bladder) and was operated and then 3 yrs later again he developed bilateral loin pain and was diagnosed with bilateral renal calculi (ureters) and was operated with a gap of one month. Then 3 yrs later he developed a midline swelling approx 3 cm below umbilicus and diagnosed with abdominal hernia and a mesh was placed. Then 1 yr later he developed abdominal pain and endoscopy was done and diagnosed with ulcer and was managed conservatively During this he was diagnosed with diabetes and hypertension Then 2 y
